This license is for one installation on a single MS Windows Pocket PC or, if purchased, one desktop PC or tablet computer, for one doctor at one address.

THE SOFTWARE SUPPLIED HEREIN IS PROVIDED TO YOU UNDER THE EXPRESS CONDITIONS THAT YOU AGREE WITH THE SOFTWARE LICENSE. BY OPENING THE FACTORY SEALED DISKETTE PACKAGE AND/OR USING THE SOFTWARE, YOU AGREE TO THE PROVISIONS HEREIN WITH NO REFUNDS APPLICABLE. IF YOU DO NOT AGREE TO THE PROVISIONS HEREIN, PROMPTLY RETURN THE UNOPENED DISK PACKAGE INCLUDING ACCOMPANYING MATERIALS AND PACKAGING FOR A FULL REFUND LESS A 15% RE-STOCKING OR PERSONALIZATION CHARGE. NO REFUNDS ARE APPLICABLE FOR ANY PURCHASE WHERE 30 DAYS HAS TRANSPIRED FROM THE DATE OF PURCHASES.

This software and any corresponding diskettes on which it is contained (the "Licensed Software"), is licensed and personalized to the purchaser ONLY, the end user, for one office site only, for your own use. The software may only be used on one computer (i.e., a single CPU) at the same time, unless you have purchased a network version. You do not obtain title to the Licensed Software or any copyrights or proprietary rights in the Licensed Software. This license allows full use of the software as long as payment obligations are fulfilled. For users with balance due or Trial Pak, the license extends only through the next payment due date or to the end of the trial period, respectively. After receipt of payment, the user will periodically receive "license extension diskette(s)" to extend the license through to the next payment due date. If payments become delinquent, the license to use the software is suspended and support will be withheld until the account becomes current. In addition, the software itself may cease operation (without loss of data), in which case, the "license extension diskette(s)" will be required. You may not rent or lease the software to others. You may transfer the license to another only by EMR4Doctors LLC re-licensing and re-personalizing the software to the other party, and only if the receiving party agrees to the terms of this Agreement. Under these conditions you must transfer the originals and all copies retaining no copies for yourself. You may not transfer, convert to another programming language, decompile, disassemble or otherwise reverse engineer the Licensed Software for any purpose. EMR4Doctors LLC reserves the right to refuse support or service on products deemed to be obsolete or no longer in production.

DO NOT MAKE ILLEGAL COPIES OF THIS SOFTWARE. INDIVIDUALS WHO MAKE UNAUTHORIZED COPIES OF SOFTWARE MAY BE SUBJECT TO CIVIL AND CRIMINAL PENALTIES. PLEASE REPORT ANY ENCOUNTERED UNAUTHORIZED COPIES TO EMR4Doctors LLC IMMEDIATELY.

The LIMITED WARRANTY contained herein limits EMR4Doctors LLC's liability. In no case shall EMR4Doctors LLC's liability exceed the license fee paid for the right to use the licensed software.

This license constitutes the entire agreement and understanding between the parties and supersedes any prior agreement or understanding whether oral or written, relating to the subject of this license. This Agreement may only be modified by a written agreement signed by EMR4Doctors LLC. COPYRIGHT 2009, All Rights Reserved.
